76 year old woman, diagnosed with hydrocehalus at age 60. she has limited mobility, has had numerous visits to drs. regarding incontenace and stomach issues, had a hernia which became a bowel blockage,aspe rated fecal matter into lungs, turned into pneumonia. that was a year ago. this past week has been complining of terrible diarrah/constipation. She is on numerous medications for evey aliment for stomach distress, as well as taking over the counter meds. she also has some type of heart issue which she takes a daily med for. concerned about all the medications and there possible inrteractions with OTC's and each other. looking for a advocate to evaluate her medication intake. Would like to find a third party that would not be connected with her current doctors.

I would start by asking do you have a HIPAA authorization to get information, if not, who does? Someone could go over all meds with her pharmicist (hopefully she's getting all meds at one place) to check for interactions. More than that, she needs one Geriatrics doctor coordinating her care. Is she in a facility? It sounds as though she is also self medicating, which could be a big part of the problem. So the short answer is, write everything down that she's taking and call her pharmicist. Then find her a good geriatrician.
