After three years in a large, fancy facility we are moving our mom to a small group home. Also, mom is extremely in continent with bad odor. What can we do about the odor? And how can we help?

If she is being cared for properly she shouldn't smell, at least not all the time! Do they not bathe her? Is she not changed when she is soiled?
Strong urine odour could be from not drinking enough or from a UTI. Body odour could be from improperly cared for skin folds and creases and below her breasts. There may also be wounds/pressure sores. Have you checked her over?

It would really help to know your mother's health status, and why she is being moved.

What is your mom's condition? Is she immobile? Does she have dementia? Why is she being moved from the large facility? Was it a nursing home, Memory Care, etc?
