My mother passed away September 21,2020 under the care of the owner, Vicky Carlos/Theobald. •Vicky didn’t call anyone earlier regarding Mom’s deterioration two days before her death. She never called hospice when Mom was failing. The state mandates that all facilities, including board and care notify the hospice agency, doctors and the family of ANY changes such as fever, vomiting, injury, etc. The hospice nurse, Monica was there when I visited my mom the last time September 21, 2020. Monica told me she was not notified of my mom’s decline. Vicky told my sister that “Your mom just eats a few bites then pushes her plate away.” Nothing about the fact that your mom can’t move, or talk, or can’t swallow etc. When I called Vicky on my way to visit my mom she only told me that my mom wasn't answering her phone and "checked out". I was shocked to see my mom in a comatose state when I arrived. She was struggling to breathe and did not appear to know who I was. George, Monica’s supervisor, had no idea Mom was this bad. He went through all the notes and never was told any of this. •The owner, Vicky Carlos didn’t require face masks upon me entering her senior home/care facility. I visited my mom on three separate occasions and Vicky was not wearing a face mask each time. She didn’t require me to wear one either on any occasion. She did not take my temperature and did not require that I sign a log that I did not have COVID 19 or any symptoms of COVID 19. Vicky cut my mom's hair without her
