SYNERGY HomeCare understands that the first step in determining what type of care is most appropriate is to meet with a client and their family for a complimentary in-home assessment and visit. During this visit, we want to learn more about how a client’s day to day activities are impacted by the symptoms they are experiencing so that together we can develop a customized plan of care for our care team to follow.
