Senior Solutions has been providing Care Management for 24 years. We have 5 Nationally Certified Professional Geriatric Care Managers through the National Association of Professionally Certified Care Managers (NAPGCM) including 3 RNs, 2 of which have have almost 14 years experience with Senior Solutions, 1 MSW with extensive Psychiatric experience, and 1 Administrator with over 16 years experience supervising care and case managers. A Care Manager is trained in providing physical and psycho-social Assessments and developing a Care Plan for the present as well as for the future of adults with disabilities or a disease and older adults. This can include a cognitive assessment or assessments of medical records, health and long term care insurance policies, and a financial assessment to determine the feasibility of a possible move into a Senior Living Facility. In addition a Care Manager can provide on-going care management and advocacy as one ages, medication management, and assistance in scheduling as well as taking clients to physician appointments. We can even recommend a physician based on one's diagnosis. Finally a Care Manager can prepare a healthcare living will, assist with family mediation or choosing an Elder Law Attorney, a Trust Officer, and/or a home care provider.

Senior Solutions can also provide individuals or couples with a non-medical home care provider from 4 to 24 hours a day or with a 2 Hour Special Visit or with a Live-In. We have been providing this Pennsylvania State Licensed Care for 22 years and can provide you with many experienced and quality providers for companionship or assistance after surgery or with help around the home.
