We advocate for patients and provide assistance between physicians, specialists for diagnostic studies, therapeutic interventions, or other services that may benefit the patient. We are extremely effective in coordinating and facilitating clients’ needs—from creating solutions-based care plans to providing and implementing referrals for legal and fiduciary assistance, geriatric care management, home health, hospice and speciality services.
Health & Wellness Care Visits
Senior visits provide support to our clients and peace of mind to their loved ones who are not able to visit daily. These visits support therapy & activity goals, medication management and oversight, wellness checks, and support with clients transitioning from home to a facility.
