But finding the right home health agency can be challenging for a number of reasons: large amounts of paperwork delay visits, nurses and aids may lack the training needed to attend to specific conditions, and doctors and case workers are often tasked with coordinating multiple agencies to deliver the appropriate treatment.
These challenges risk many patients being readmitted to the hospital.We are a home health agency built by nurses and healthcare leaders who consistently witnessed these challenges. We have also felt the pressures of being family caregivers ourselves. We are here to facilitate the transition from the hospital or clinic to home by collaborating with our community partners and ensuring it’s a smooth process.
