Our high-touch, team-based medical care and social services enable our participants to enjoy their life to the fullest as they age in their own preferred place, on their own terms, and where they feel the most comfortable.
We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place. We are a steward of the public resources entrusted to us and we hold ourselves accountable for coordinating health care in ways that make it more affordable in addition to providing excellent quality and service.
