Homewatch CareGivers of the Lehigh Valley is a licensed Home Care Agency providing Home Care to the Lehigh Valley for over eleven years. The service begins with an assessment of the Elder’s needs and the creation of a Plan of Care, which is a suggested plan of action. This can be particularly helpful when family members disagree on what the Elder needs or simply don’t know where to begin.

The assessment covers many areas of the Elder’s life, for example the condition and safety of the home, medication management, and psychosocial needs. The Care Manager can make referrals to community resources and local professionals.

Our Professional Care Managers can provide ongoing oversight of the Elder’s In-Home services, household management and general wellness. They can coordinate the Elder’s healthcare needs by tracking and scheduling appointments with their physicians and other healthcare professionals, arranging for transportation to and from appointments, monitoring follow-up care needs and communicating the Elder’s condition to the family members.

Our Professional Care Managers can also assist with Long-term Care Planning, End of Life Planning, Document Organization and Transitional Assistance when moving to a new home.
