Our care team of nurses, aides and therapist work together to help patients reach the highest level of independence and wellness. Our treatment goals are always to keep patients at home where they are most comfortable and out of the hospital.
Home Care Options' highly skilled nurse will provide a thorough patient assessment. From this assessment and in consultation with the patient, physician and family members; an individualized treatment plan is developed that is tailored to the needs of the patient. Therapy assessments are conducted for the clients who require rehab due to weakness, or status post surgical procedure.
Our services are provided primarily by home health aides through shift work (4-hours or more), and are available 24 hours a day, 7 days a week. Our caregivers excel at what they do, and their assistance can help adults and seniors continue to live independently at home.
