Our commitment to quality care begins with our compassionate and proactive approach to care. This process begins with identifying clients’ and families’ unique needs, creating a patient-centered care plan, coordinating with physicians and other providers, and implementing quality assurance throughout the duration of care. We have a rigorous 10-step screening process for our caregivers, nurse oversight of client care, and ongoing training and evaluation at every level.
ComForCare specializes in providing personalized non-medical home care. Non-medical care, sometimes referred to as “home care” or “personal care,” includes a wide range of elder care support and in-home assistance by a non-licensed provider. We provide the day-to-day care that your loved one may need instead of or in addition to care by a nurse or other health professional.
Whether it’s due to injury, illness or aging, sometimes older adults need a little extra help, especially at home. However, family members and friends can’t always be there 24/7 to assist their aging loved one. That’s where ComForCare Home Care comes in. Our personalized care plans and compassionate caregivers help seniors live independently in their own home and continue to do all the things they love.
Services are available around the clock, 24 hours a day, seven days a week, including holidays. We can provide same-day home care, transition-of-care, such as transportation home after a discharge, or assist with many other needs that arise when a family member is in crisis. We also provide support to hospitals and care facilities, including staffing hospital sitters if your loved one needs an extra level of attention.
Our highly-trained staff are ready to talk with you about your needs and answer your questions. Once we understand your situation, we’ll provide you with elder care options for your loved one, describe our services in detail, and schedule an in-home visit to conduct a thorough needs evaluation.
We proudly serve the following communities in St. Paul East: Afton, Bayport, Cottage Grove, Lake Elmo, Lake Saint Croix Beach, Lakeland, Lakeland Shores, Landfall Village, Little Canada, Maplewood, Newport, North St. Paul, Oak Park Heights, Oakdale, Pine Springs, St. Croix Beach, St. Mary's Point, St. Paul, Stillwater, West Lakeland, West St. Paul, and Woodbury.
We will take time to listen to your concerns. Once we have a better understanding of your needs, we'll share care options, provide further details, and schedule a home visit to conduct a needs evaluation.
We give families the opportunity to get to know us better during the in-home visit. One of our staff members will evaluate your loved one's needs, as well as those of your family. We will set goals for care, perform fall prevention and home safety checks, and discuss costs and funding options.
Once the level of care has been agreed to, one of our experienced nurses will visit to perform a complete assessment of your loved one’s physical, mental and social needs. The nurse will use the assessment to develop a personalized care plan that will be followed by the caregiver at every visit.
Matching Caregiver to Care Recipient
