ComForCare specializes in providing personalized non-medical home care. Non-medical care, sometimes referred to as “home care” or “personal care,” includes a wide range of elder care support and in-home assistance by a non-licensed provider. We provide the day-to-day care that your loved one may need instead of or in addition to care by a nurse or other health professional.
Our in-Home Care Services are available around the clock, 24 hours a day, seven days a week, including holidays. We can provide same-day home care, transition-of-care, such as transportation home after a discharge, or assist with many other needs that arise when a family member is in crisis. We also provide support to hospitals and care facilities, including staffing hospital sitters if your loved one needs an extra level of attention.
What to Expect
Dan Patterson, President/Owner and the dedicated ComForCare Team will take time to listen to your concerns. Once we have a better understanding of your needs, we will share care options, provide further details, and schedule a home visit to conduct a needs evaluation.
We give families the opportunity to get to know us better during the in-home visit. One of our staff members will evaluate your loved one's needs, as well as those of your family. We will set goals for care, perform fall prevention and home safety checks, and discuss costs and funding options.
Once we agree upon to the level of care, one of our experienced nurses will visit to perform a complete assessment of your loved one’s physical, mental and social needs. The nurse will use the assessment to develop a personalized care plan that will be followed by your caregiver at every visit.
