Providing Care Management and Consultation Services:
Care Management is for older adults who are experiencing health and lifestyle changes. We complete a comprehensive assessment which has recommendations for services and resources to best assist them. We then arrange for, monitor and modify the services as needed. We are available to assist in an emergency. We partner with family and caregivers to meet the needs of the client on an ongoing basis.
Consultation is for those older adults who are thinking about future changes, and would like to know more about their options. We complete a comprehensive assessment based on their finances, lifestyle and health, and provide a recommended plan that takes into consideration the myriad of services available. This is a short term service usually involving 1-3 visits to complete.
