Careplan Geriatric Care Managers is a independent group not affiliated with any hospital or healthcare system; for this reason your loved one will get the best unbiased assessment and recommendations. With over 20 years of geriatric care experience, Careplan has the knowledge and community resources to help keep your loved one safe at home or provide assistance with nursing home or assisted living placement. The mission of Careplan is to keep the elderly person aging at home for as long as it is safe and possible when living at home is no longer the best option Careplan can help facilitate placement at a senior living community. Our goal is also to prevent repetitive hospitalizations. Let Careplan be the family liaison assisting on behalf of those families that live to far or close. We can Asses, Supervisor and Implement a plan of care to help keep your loved one safe and healthy at home, for as long as possible.

Here is What Careplan can do for you:
Conduct care plan assessments to identify problems and provide solutions for you.
Screen arrange and monitor in home services for you.
Provide short or long-term assistance for caregivers living near or far away.
Review financial, legal or medical issues and offer referrals to geriatric specialists.
Provide Crisis intervention
Act as a liaison to distant families and provide oversight of care. Alert when their is a problem.
Assist with moving an older person to or from a retirement complex, assisted care home, or nursing home.
Provide consumer education and advocacy
Offer counseling and support.

I've referred several of my friends to Bridget for assistance with their elderly parents. They couldn't thank me enough for the referral. Prior to their Careplan meeting they were not aware of any service providers that provide this kind of service. She exhibits unparalleled expertise, compassion and professionalism in attending to the needs of each of her clients and their extended family members. Bridget has an abundance of resources, contacts and services that is truly a benefit for families. As a nurse having experienced both hospital and home care settings, I cannot underestimate the importance of a knowledgeable and compassionate care manager. I have experienced many cases where the current home situation is no longer safe, healthy, or appropriate because of declining physical and/or mental health. Knowing where, when and who to turn to can be difficult.Bridget has made that transition much easier for many families. Careplan Geriatric Care Managers helped with guiding my mother and father-in-law from their home to an assisted living. Our geriatric care manager met us in the hospital after my father-in-law was diagnosed with heart failure. My mother-in-law had Parkinson's and could not be left alone. Bridget Ritossa and Careplan Geriatric Care Managers found reputable in-home help for Mother while Father was in the Cleveland Clinic. Bridget worked with the hospital discharge planner and our insurance to find the best care community for us, taking into consideration our finances and lifestyle. Currently my parents are both living together in a life care community in Cleveland that offers all I could want for them. Great care, social activities and peace of mind for me and my siblings.
