We will complete a comprehensive assessment in order to help the senior and their family determine the best course of action for having the seniors changing needs met rapidly and efficiently. We evaluate clients where ever they are, either at a hospital, home, or assisted living. We assess their cognitive, physical, medical, environmental, psycho-social supports, financial ability and legal concerns to determine if the client can remain or safely return home. We create a detailed plan of care, for both long and short term concerns.We will coordinate things like respite care, Assisted living, home care support, end of life care planning, hospice, and palliative care. We also refer you to caregivers to assist with everything from personal care, to bill paying and escorting to medical appointments. We provide crisis intervention and conflict resolution to families who may be struggling to reach a consensus regarding difficult decisions for the senior.

Answers for Elders also collaborates with affiliated legal advisors and financial experts for Mass Health Medicaid planning as well as preserving life savings, (protecting the maximum amount of assets allowed by law). We can also assist in preparation of documents such as, power of attorney, health care proxy and HIPPA medical release forms for comprehensive continuum care and services.
