Our big, audacious goal is to ensure that every complex, continuous care patient able to live at home, has the Care Team and funding system available to them, to live safely at home. We are committed to surrounding the hundreds of people in CT who are currently living in hospital beds, rehabilitation facilities, and nursing homes with the quality Care Teams needed to support them at home.
