Our Care Managers are health and human service professionals, with a specialized body of knowledge and experience related to aging, elder care and special needs issues. Our Care Managers assist older adults and persons with disabilities in attaining their maximum functional potential, and deliver care-coordination and support services with sensitivity to preserve the dignity and respect of each individual.

I am a current client

My family and I were struggling to make sense of what we needed to do when we found out our mother was diagnosed with early onset dementia when we found Specialized Care Management. The staff and our dedicated care manager showed great compassion and expertise not only to our mother but my entire family during this trying time. With our mother's health and quality of life deteriorating, they provided us options and solutions I was genuinely astounded at the caliber of care and professional knowledge of the team at Specialized Care Management. Should you need this kind of assistance with a family member, you can not do any better than this group of dedicated, competent and kind professionals.
