Eldercare Specialists was founded in 2005, by Lynn Goehner, BS, RN, PHN, CCM, to provide geriatric care management services, in the San Francisco, Marin, Contra Costa, Sonoma and Alameda counties. We provide caring case management services, case management solutions and family counseling to support families and older adults as they age and are faced with dementia, depression, psychiatric issues or complex medical issues. Our geriatric care managers ensure our clients have the best senior care services and professionals to optimize their health and quality of life at home, in assisted living or skilled nursing.

I am a current client

Eldercare Specialists managed my parents care for the last eight years. Their care managers were excellent in helping us get the best physicians, medical treatment and care in the home. My Mom loved her care managers Marilyn and Michele, their visits were one of the highlights of the week. We had private homecare and they did an excellent job of setting up the care, overseeing the caregivers and helping us set up and process payroll. Our parent's would not have been able to stay home without their help. Thank you Eldercare!
