CMC provides elders/families with tools to live there life with dignity and quality. As a nurse case manager I will conduct a thorough assessment and develop an individuaized plan of care. I encourage the family to participate in this plan of care. Together we will identify the elders needs and what we must do to keep them safe and as healthy as possible.
