Care Navigators is a care management company composed of Registered Nurses, Social Workers and Advocates that provide a comprehensive assessment including a cognitive, medical, legal, financial and psycho-social evaluation. A life plan of care is created with appropriate referrals and resources for the client. We can assist the client by implementing the life plan for the client. This allows the family to be the son or daughter, not the caregiver for their loved one. Care Navigators will advocate for you or your loved at MD visits, hospitals, rehab or a nursing home. We can assist with non-clinical tasks such as paying your bills or clinical tasks such as ensuring all aspects of your medical care are being managed by setting up medications, coordinating and communicating care to all medical specialists and stepping in whenever help is needed.
