Can NYS medicaid long-term nursing care in a nursing home be transferred to medicaid for home care?

I am living in a long term nursing home facility on New York State Medicaid and would like to live at home under Medicaid with home care. Is that possible? How many hours would they cover? Husband lives in house, alone, has title, would like to downsize, move to a HOA maintained town house. If my medicaid can covert to homecare, I would like move to the smaller house and be cared for there. How can all of this be orchestrated?
