How do you know when someone is in the last stages of congestive heart failure?

swelling in legs, feet, shortness of breath, constant fatigue, lungs sounds are "wet"
My dads testicles swelled up like a volley ball plus his hands, feet, ankles ;the water moved around. Tongue turned black-face turned grey. Breath got more shallow and then that dreaded rattleing sound.
