How can I ensure my wife, who has Alzheimer's will remain in her home with her pets when I will no longer be here to keep her at home?

Wow, ng...your post tears at my heart. You are shouldering so much right now, and trying to look toward the future too.
Maybe the first thing to do is see an edler attorney...He or she can give you advice on estate planning, long-term care, and guardianship, etc. Do you have a loved one of family friend who could act as a trustee and oversee your wife's care in your absence?
This is a heavy time...but bless you for being a loving spouse.
