Comprehensive Care Management (CCM,) a member of the Beth Abraham Family of Health Services, a not-for-profit 501(c)(3) corporation, is nationally recognized for its scope and innovation of long-term care services for chronically ill and disabled adults.
CCM includes PACE, the nation’s largest Program of All-Inclusive Care for the Elderly, which integrates medical care, social day care and supportive care for the frail elderly; CCM Select, a Medicaid Managed Long-Term Care Program; and CCM Direct, a Medicare Advantage Special Needs Plan.
