Most states have a Medicaid Waiver program, wherein the Medicaid patient can choose a caregiver to enable them to stay in their own home (I think it's called "The Money follows the client". They have a triage system to determine the level of need for each client. This may be worth a try - not that the pay will be that great! Call your local Council on Aging for more info.
